When you want to shift gears, depress the clutch.
When you want to stop or drastically slow down, press down on the brake. You only need to depress the clutch if your braking caused you to slow enough that you are now in too high of a gear.

Brake and the clutch, your instructor is right
You would fail your driving test for doing it the other way around

No your instructor is right, you brake first and then clutch down just before you feel the car will stall.

brake then clutch. you don't have to push in the clutch every time you hit the brakes. depends on how fast you are going

first brake (slowly) then clutch to stop. but to shift gears sometimes just clutch is ok, depending on the speed you're driving. if you need to slow down before shifting gears then you need to brake a bit first.
